Determination of Serum Bilirubin

1. Aim

To review the measurement of total serum bilirubin using an authenticated diazo method and record the quality checks required before a result is reported. This is experiment 15 in the PCI Medicinal Biochemistry practical list. The available college manual does not provide a dedicated serum bilirubin procedure, so the exact teaching assay must follow your college lab manual and instructor's method sheet. [1, 2, 3]

2. Requirements

Use a labelled serum control or deidentified specimen, a validated total-bilirubin assay kit or laboratory method sheet, calibration and quality-control material, pipettes or analyzer, light-protective specimen handling, gloves, coat, eye protection, and biological and chemical waste containers. The method's reagents, specimen amount, wavelength, reaction timing, calibration, reportable range, and acceptance criteria must follow your college lab manual and instructor's method sheet. No student blood collection is implied. [2, 3]

3. Theory

Bilirubin is produced as older red blood cells are broken down. Blood tests may report total bilirubin and, when ordered, a direct fraction. The difference is commonly described as indirect bilirubin, but its calculation requires results obtained by compatible validated methods. A bilirubin result alone cannot identify the cause of jaundice. Age, clinical context, sample condition, and the laboratory's reference information matter. [3]

A CDC laboratory method describes a timed diazo reaction for total bilirubin in serum or plasma. Bilirubin forms a colored product with a diazo reagent, and the measured change in absorbance is related to concentration by the analyzer's calibration. This principle does not define a universal tube recipe. The linked CDC protocol gives one instrument-specific example; the teaching laboratory's actual method and controls must be confirmed. Protecting the specimen from unnecessary light exposure and checking for hemolysis or other interference belong to the local quality procedure. [2]

4. Procedure

  1. Confirm the PCI experiment title and the absence of a dedicated method in the college manual. Obtain an authenticated total-bilirubin assay procedure before any physical work. [1, 2]
  2. Verify specimen identity, matrix, collection time, storage, light protection, and visible condition. Enter missing details Record here. Treat all human serum as potentially infectious. [2, 3]
  3. Read the method's calibration, blank, control, timing, wavelength, and interference requirements. Record its version and reagent lot. [2]
  4. Under supervision, process the controls and specimen only by that approved method. Do not substitute volumes from another diazo kit. [2]
  5. Record the method-defined blank or baseline and final response, then check calibration and control acceptance before calculating a result. [2]
  6. If a separate direct bilirubin result is supplied, confirm that the laboratory allows subtraction to derive an indirect fraction. Do not assume direct and total results from unrelated methods are interchangeable. [3]
  7. Report the observation, quality-control status, and result only if all requirements are met. Route any unusual patient value through the clinical laboratory, not this teaching record. [2, 3]

5. Calculation, graph, or images as appropriate

Record item Required entry Status
Serum identity and handling Specimen record Record here
Assay method, calibration, and control Local approved method Record here
Initial and final optical response Instrument record Record here
Total bilirubin result and unit Calibrated output Record here
Direct bilirubin, if ordered Compatible measured result Record here
Derived indirect bilirubin Total minus direct, if allowed Record here

Worked calculation structure: A timed endpoint uses the method-defined final reading minus its initial reading to obtain the response change. A blank or calibration correction is then applied according to the instrument protocol. If compatible total and direct results are available, the indirect fraction is total minus direct. No numerical concentration can be calculated because no specimen readings or calibrated factor have been supplied. [2, 3]

Serum bilirubin measurement review

6. Result

Report the assay identity, measured or supplied total-bilirubin value, units, and quality-control status only when documented. Until then, the concentration and interpretation must follow your college lab manual and instructor's method sheet. This teaching review is not a patient report. [2, 3]
